Description:

Choices is a Medicaid Waiver option available only to enrolled PASSPORT Consumers. The Choices Program gives the Consumer more control over the management and delivery of their services. This is referred to as consumer-directed care. In the Choices Program the Consumer is actually the "employer of record."

Requirements for PASSPORT Consumers interested in Choices Program include:
- Consumer must live in the community (not in NF, RCF, Group Home, etc).
- Consumer must be willing and capable of directing services, as demonstrated by:
- Attending all training
- Possess skills to supervise, select and monitor service providers; perform record-keeping; develop care plan; understand requirements for provider services; develop a back-up plan; work with Care Manager in all of the above.
- Successfully complete post-test.
- Must have physician approval.
- Otherwise, a Consumer may appoint an Authorized Representative to act as the employer on their behalf.

The Authorized Rep cannot be a paid provider.

Authorized Representative must demonstrate the following:
- Be willing to act on Consumers behalf
- Be committed to Consumer
- Agree to visit Consumer at least every pay period
- Meet all program requirements for CHOICES
- Submit to criminal background check, if requested
- Complete a screening questionnaire


Consumers may hire a Home Care Attendant directly. Another word for Home Care Attendant is Individual Provider (IP). This may be family members, with some exceptions, or friends, neighbors, etc.
Requirements for IP:
- Must be at least 18 yrs with valid SS #.
- Must complete application for ODA.
- Must be Medicaid-certified (obtain MA provider number).
- Must provide proof of automobile insurance.
- Must complete a Criminal Background check.
- Must sign and abide by the "Employee Code of Ethics."
- Perform all duties in accordance with OAC 173-39-02 Conditions of Participation.

- Escort/Transportation
- Heavy Household Chores
- Yard Work
- Snow Removal
- Money Management
- Home Delivered Meals
- Therapeutic Meals
- Alternative Meals
- Adult Day Service
- Emergency Response Service
- Minor Home Modification
- Durable Medical Equipment
- Pest Control
